Assistance finding community programs
Our social service staff can help you to find support programs available for families in your situation. Our trained social workers are a vital component of our home care program. Our goal is to improve or maintain the social, emotional, functional and physical health status of each patient while enhancing the coping skills of the family and other caregivers. Sometimes the difference between staying at home and having to go to a facility is as simple as periodic visits from a nurse. Our nurses teach and advise the patients and family members, coordinate the treatment plan with other team members and communicate with the patient's physician. We are committed to providing the highest quality and most cost-effective care for each of our patients.

Our physical therapists can make visits to your home to help your loved one regain strength and the ability to function as independently as possible. Based upon a complete evaluation, our physical therapists develop a personalized and therapeutic rehabilitation program, in conjunction with the patient’s physician, and use a variety of modality and treatment techniques.

If your loved one has had a stroke or other condition that makes communication difficult, we can help. Our trained speech therapists provide corrective and rehabilitative treatment to our patients. Based upon a complete evaluation, our speech therapists develop a personalized and consistent rehabilitation program, in conjunction with the patient’s physician, and use a variety of treatment techniques.
