About Us: Senior Care Connections
Senior Care Connections was founded in 2003 by Sarah Parran, MSSA, LISW-S, CMC to offer comprehensive care management services to seniors and people with disabilities and their families throughout Northeast Ohio. As an independent, professional care management company, we provide both short and long term care management services to our clients by offering access to a variety of high quality service providers who are dedicated to meeting the unique needs of our clients. Senior Care Connections has been trusted by hundreds of clients and their families; providing family consultations, comprehensive assessments, care coordination and advocacy services to vulnerable seniors and others with physical disabilities. Our professional care managers are licensed, insured and focused only on the best interests of our clients. We have developed excellent working relationships with a wide array of high quality, professional providers in the community. In addition, we work with all existing care providers and advisors used by our clients to ensure continuity of care. We do not receive any compensation for referrals and our company only provides care management services to clients to eliminate any potential conflicts of interest in recommending specific types of services or providers. You can feel comfortable that our focus will always only be on what’s best for you and your family.
About Sarah Parran, MSSA, LISW-S, CMC
Senior Care Connections was founded by Sarah Parran, MSSA, LISW-S, CMC, a master's degreed social worker with over 25 years experience in geriatric and health care social work. Sarah is a licensed independent social worker with supervisory designation and a care manager certification. She is a member of the adjunct faculty of the Case Western Reserve University Mandel School of Applied Social Sciences, teaching an advanced health practice class to social work graduate students since 1994. In addition, she serves on several boards of directors of Northeast Ohio not-for-profit agencies. Prior to founding Senior Care Connections, Sarah served as the Director of Social Work at University Hospitals of Cleveland for 12 years. Sarah has expertise in building client rapport, managing collaborative interdisciplinary client services, effective communication and coordination of care within complex medical environments.
She has developed a national reputation for her:
- Expertise regarding healthy aging, Alzheimer's disease and dementia care, health care systems, community resources, care planning and coordination
- Extensive experience dealing with health care, legal and financial providers
- Commitment to promoting successful aging strategies and supporting maximum independence for older and disabled adults
