Information and Referral
We offer an initial complimentary consultation. As we review your situation and concerns, we may find that there are other services or care options that are better suited to your needs. We are very familiar with the providers and support systems here in New Hampshire that help families in need. If you are calling about a loved one who lives outside our service area, we may refer you to colleagues who can help.

When making decisions about care for a loved one, the pace of decision making can be fast and furious. At AgeQuest, understand the process, and can act as a sounding board or coach. We help you articulate your questions and concerns and gather the information you need before making care decisions. We also can facilitate conversations where there may be a difference of opinion about where to start.
Health Care Advocacy
Providing support for an older person in today’s fast-paced health care system can be a daunting experience. Our professional team can provide a comforting, knowledgeable presence in a variety of health care settings. We go with our clients to doctor visits, meet them in emergency rooms, and provide continuity through our visits to hospitals, nursing facilities and home.

Assessment is the foundation of geriatric care management. Through this objective process, we work with you first to make sure we understand your questions and concerns. The scope of an assessment can be focused on a specific goal (such as helping with transition from hospital or nursing facility to home) or more comprehensive in nature.
Care Monitoring and Support
AgeQuest provides ongoing professional monitoring of individuals living in a variety of settings, including their own home, retirement communities, assisted living, residential care and nursing homes. This can be especially helpful when family members live out of town, or when a guardian or conservator is responsible for overseeing care.

AgeQuest helps clients develop ongoing plans of care. As we work together to develop the plan, we help clients understand provider options, costs, and where they might look for help if they are struggling to pay for care.
Ongoing care management may include making regular home or facility visits, making needed referrals, training caregivers, accompanying to health care appointments and maintaining close contact with family members.

Family Caregiver Education and Support
As part of our quest to develop savvy, confident consumers and compassionate, capable providers, AgeQuest offers a variety of educational programs. Look for our offerings in our monthly e-newsletters, or request further information on particular programs of interest.
We are now offering "Making Sense of Memory Loss" - a six part series for family caregivers of individuals with early stage Alzheimer's disease or other dementia. Classes are held weekly on Tuesdays from 4-6pm.
